What to expect before during and after the visit to see the doctor.  Your doctor may not ask for all of this information, but bring as much as you can and it will be helpful to our staff. 

  • CD or DVD of your imaging studies

    If you have had imaging studies such as X-rays, CT Scans, or MRIs please bring the CD or DVD of the study with you. It is very important that we have this for your clinic visit.

    If your imaging study was completed at UHN (TGH, TWH, PMH) or Mt Sinai or Women’s College hospital, it is not necessary to bring the CD or DVD with you.

  • List of all medications you are currently taking

    This includes prescription medications, over-the-counter medications, vitamin or mineral supplements and herbal remedies.

  • Medications that you need to take during the day

    This includes prescription medications, over-the-counter medications, vitamin or mineral supplements and herbal remedies.

  • Record of any changes

    Make notes about any changes in your condition that you’ve notice since your last visit, and bring these notes to show your doctor. Also bring any medical equipment you use, such as your colostomy bag, if you have one.

  • A trusted friend or family member

    A friend or family member can give you emotional support and can help you make good choices. They can also help you gather information, take notes and ask questions.  Family is always welcome to attend Clinic visit with the patient especially when surgery may be required.

  • List of doctors and hospitals

    Bring a list of doctors you have seen in the past and who may have information about your medical health. Also bring the names of the hospitals where you have been treated.

    Bring a paper copy of recent blood work, especially creatinine.

  • Questions to ask

    Bring a list of questions to your appointment to help you remember everything you want to ask.

Please check in at the front desk We will review your appointment details and your insurance coverage with you. 

Appointment lengths can vary, depending on the complexity of your visit. If you have any specific concerns relating to your schedule, please call the office before you arrive. 

It is a good idea to take notes of anything the doctor or the staff tells you to do at home.
